At the end of your patient’s appointment:
• Write your name at the top of the page
• Check off recommended items such as toothbrush, floss, or other oral care products
• Add any personalized notes or recommendations for home care
• Include the next recare visit recommendation if needed
• Tear off the sheet and give it directly to your patient as their take-home instructions

How to Properly Tear Your Pad
To keep your OHI pad neat and intact, follow these simple steps:
• Hold the pad firmly with one hand near the top edge
• Gently bend the page slightly backward along the perforation
• Slowly tear side to side, not straight down
• Avoid pulling quickly or at an angle to prevent tearing into the next sheet
